She cycles through irritability and sadness in ways that feel foreign to her own history. She visits her primary care physician and leaves with a prescription for an antidepressant and a referral to a cardiologist. Menopause is never mentioned.

This scenario is not exceptional. It is, according to a growing body of clinicians and researchers, disturbingly routine.

The United States has approximately 6,000 women entering menopause every single day. Yet a 2019 survey published in Menopause, the journal of the Menopause Society (formerly NAMS), found that fewer than 20% of OB-GYN residency programs offered a dedicated menopause curriculum. The downstream effects of that training gap are measurable in waiting rooms across the country.

What Perimenopause Actually Looks Like—and Why It Confuses Clinicians

The term "menopause" is clinically precise: it refers to a single point in time, defined as 12 consecutive months without a menstrual period. What most women actually experience—sometimes for 7 to 10 years preceding that marker—is perimenopause, a hormonally volatile transition phase that can produce a remarkably wide symptom profile.

Vasomotor symptoms such as hot flashes and night sweats are the most recognized. But the neurological and psychological dimensions are frequently underappreciated. Estrogen receptors are distributed throughout the brain, including in regions governing mood regulation, memory consolidation, and sleep architecture. As estrogen levels fluctuate and eventually decline, the consequences can include cognitive fog, heightened anxiety, disrupted sleep, and depressive episodes—even in women with no prior psychiatric history.

The diagnostic confusion arises because these symptoms overlap substantially with generalized anxiety disorder, major depressive disorder, and hypothyroidism. A clinician who does not take a thorough menstrual history—or who does not recognize that irregular cycles can precede menopause by years—may plausibly interpret the presentation as a primary psychiatric condition. The patient is not poorly served through negligence; she is poorly served through a structural gap in clinical education.

The Research Deficit That Shaped a Generation of Care

The modern landscape of menopause medicine was profoundly shaped by a single study: the Women's Health Initiative (WHI), whose preliminary findings were released in 2002. The trial, which examined the effects of combination hormone therapy in postmenopausal women, was halted early after interim analyses suggested elevated risks of breast cancer, heart disease, and stroke.

The headlines were alarming, and hormone therapy prescriptions dropped precipitously almost overnight. What the headlines often omitted was critical context: the WHI studied a specific formulation (conjugated equine estrogen combined with medroxyprogesterone acetate) in women whose average age at enrollment was 63—more than a decade past the menopausal transition for most participants.

Subsequent reanalysis of the WHI data, along with independent studies, has substantially refined the picture. The "timing hypothesis," now well-supported in the literature, suggests that initiating hormone therapy within 10 years of menopause onset or before age 60 carries a materially different risk-benefit profile than initiating it in older, more distant postmenopausal women. For many women in the perimenopausal window, current evidence supports that hormone therapy can reduce vasomotor symptoms, improve sleep quality, lower fracture risk, and may carry cardiovascular benefit when started early.

This is not a blanket endorsement. Women with a personal history of hormone-receptor-positive breast cancer, active cardiovascular disease, or certain clotting disorders face a different calculus. The point is not that hormone therapy is universally appropriate—it is that the conversation deserves to happen, with evidence, rather than being foreclosed by two-decade-old headlines.

The Psychiatric Misdiagnosis Pipeline

When menopause symptoms manifest primarily as mood disturbance or anxiety, the path to a psychiatric label can be swift. Research published in the Journal of Women's Health has documented that women in perimenopause are disproportionately likely to be prescribed antidepressants without a formal psychiatric evaluation. Some of these women do have comorbid depression—perimenopause is a genuine period of elevated vulnerability for mood disorders—but others are experiencing neurobiological effects of hormonal fluctuation that antidepressants alone are poorly positioned to address.

This matters clinically. A woman whose sleep disruption, cognitive symptoms, and mood instability are driven by estrogen volatility may experience partial improvement on an SSRI, reinforcing the psychiatric framing, while the underlying hormonal picture remains unaddressed. Years can pass before a menopause-informed clinician enters the picture.

The thyroid overlap adds another layer of complexity. Hypothyroidism shares several symptomatic features with perimenopause—fatigue, weight changes, mood shifts, cognitive slowing—and TSH testing is often the first workup ordered. When thyroid panels return within normal range, some clinicians effectively close the diagnostic inquiry, rather than proceeding to a hormonal evaluation.

What Evidence-Based Menopause Care Actually Involves

An evidence-informed approach to menopause begins with a thorough history: menstrual pattern changes, symptom timeline, sleep quality, cognitive function, mood, sexual health, and bone density risk factors. Laboratory evaluation may include FSH and estradiol levels, though these can be variable during perimenopause and should be interpreted alongside clinical presentation rather than in isolation.

The Menopause Society's most recent clinical practice guidelines support hormone therapy as the most effective treatment for vasomotor symptoms in appropriate candidates, and note that for women under 60 without contraindications, the benefits generally outweigh the risks. Non-hormonal options—including certain SSRIs, SNRIs, gabapentin, and the recently FDA-approved neurokinin B antagonist fezolinetant—offer alternatives for women who cannot or prefer not to use hormones.

Beyond pharmacology, lifestyle interventions carry meaningful evidence. Regular aerobic exercise has been shown to reduce hot flash frequency and improve sleep and mood outcomes. Cognitive behavioral therapy adapted for menopause (CBT-M) has demonstrated efficacy for both vasomotor symptoms and associated psychological distress in randomized controlled trials.

Finding a Provider Who Is Prepared to Help

For American women navigating this landscape, the practical challenge is finding clinicians with current menopause training. The Menopause Society maintains a searchable directory of certified menopause practitioners at menopause.org. Telehealth platforms specializing in women's midlife health have also expanded access, particularly in rural areas where specialist availability is limited.

Preparing for appointments with a documented symptom timeline—including menstrual irregularities, onset dates, and prior diagnoses—can help redirect clinical conversations toward a more complete picture. Asking directly whether hormonal factors have been considered is a reasonable and appropriate question.

The gap in menopause care is not inevitable. It is a product of historical underinvestment, outdated training curricula, and the misapplication of research findings that have since been substantially revised. Women who understand this context are better positioned to advocate for evaluations that reflect what the current evidence actually supports.
