A Growing Market Built on an Incomplete Premise

The adaptogen and cortisol-support supplement category is now a multi-billion-dollar segment of the US wellness industry. Walk into any pharmacy, natural grocer, or scroll through any health-focused social media feed, and the messaging is consistent: stress is the enemy, cortisol is the villain, and the right botanical blend will restore balance. For a subset of patients, these products do provide measurable benefit. For a larger group, however, the results are underwhelming at best and counterproductive at worst.

The reason is not that the science behind adaptogens is entirely without merit. Compounds such as ashwagandha (Withania somnifera), rhodiola rosea, and phosphatidylserine have peer-reviewed evidence supporting their influence on the hypothalamic-pituitary-adrenal (HPA) axis under specific conditions. The problem is that "specific conditions" is doing enormous work in that sentence — and most supplement marketing never mentions it.

When a patient's stress response is chronically dysregulated, the dysregulation is rarely a standalone phenomenon. It is almost always a downstream consequence of something else. Supplementing around that upstream driver is, at best, a temporary patch and, at worst, a reason to stop looking for the actual cause.

The HPA Axis Is a Messenger, Not the Message

The HPA axis — the communication loop between the hypothalamus, the pituitary gland, and the adrenal glands — is the body's primary hormonal stress-response system. When the brain perceives a threat, corticotropin-releasing hormone (CRH) triggers a cascade that ultimately results in cortisol secretion from the adrenal cortex. Cortisol then mobilizes glucose, modulates immune activity, and sharpens alertness before a negative feedback loop signals the system to stand down.

This system is exquisitely designed for acute, time-limited stress. What it is not designed for is continuous activation driven by factors that have nothing to do with psychological pressure.

Chronic low-grade inflammation, for instance, is one of the most potent drivers of persistent HPA activation. Pro-inflammatory cytokines — particularly interleukin-6 and tumor necrosis factor-alpha — directly stimulate CRH release, keeping the cortisol signal elevated even when a patient reports no identifiable life stressors. Similarly, blood sugar instability creates repeated cortisol surges throughout the day, as the adrenal glands are recruited to raise glucose levels during hypoglycemic dips. Sleep deprivation, even mild and cumulative, disrupts the natural diurnal cortisol curve, often flattening the morning peak patients need for energy while elevating evening levels that interfere with recovery.

In each of these scenarios, the cortisol elevation is not a malfunction. It is the body performing exactly as designed in response to a genuine physiological signal. Giving a patient phosphatidylserine to blunt cortisol output in this context is analogous to disabling a smoke alarm because the beeping is inconvenient.

Why Adaptogens Can Worsen the Picture

Adaptogens are broadly defined as compounds that help the body maintain homeostasis under stress. Their mechanisms vary: some modulate glucocorticoid receptor sensitivity, others influence serotonin and dopamine pathways, and still others appear to act on the hypothalamus directly. In clinical research, these effects have been observed predominantly in populations experiencing genuine psychological stress loads or early HPA dysregulation without significant underlying metabolic pathology.

When patients with unidentified inflammatory drivers, insulin resistance, or disordered sleep architecture begin taking these compounds, several things can go wrong.

First, by partially attenuating cortisol output, some adaptogens reduce the very signal the body is using to compensate for an underlying problem. A patient with reactive hypoglycemia who takes an adaptogen that blunts the adrenal response to low blood sugar may experience worsening cognitive fog, fatigue, and mood instability — symptoms they may attribute to the supplement not "working," when in fact the supplement is interfering with a necessary compensatory mechanism.

Second, there is the problem of masking. Patients who experience even modest symptom improvement from adaptogens may conclude their stress response is being managed, reducing the urgency to investigate further. Months or years can pass before the underlying driver — an undiagnosed inflammatory condition, a thyroid abnormality, a sleep disorder — is identified. By that point, the metabolic consequences may be considerably more entrenched.

Third, some adaptogens have bidirectional effects on immune function. In a patient whose HPA dysregulation is rooted in chronic immune activation, compounds that further modulate immune signaling can produce unpredictable and occasionally paradoxical outcomes.

A Framework for Evaluating the Actual Problem

The clinical question is not whether stress-support supplements work. Under the right conditions, some of them do. The more useful question is whether the patient in front of you — or the patient you are — has had the foundational variables ruled out.

Several key areas warrant evaluation before concluding that a cortisol-lowering supplement is the appropriate intervention.

Blood sugar regulation should be assessed not merely through fasting glucose but through patterns — specifically, whether energy, mood, and anxiety symptoms track with meals and fasting intervals. Continuous glucose monitoring, even used briefly, can reveal dysregulation that standard lab panels miss entirely.

Inflammatory burden is frequently underappreciated. High-sensitivity C-reactive protein (hs-CRP), along with a thorough dietary and lifestyle history, can help determine whether immune activation is contributing to HPA stimulation. Gut permeability, food sensitivities, and subclinical infections are common inflammatory drivers that go unaddressed in standard care settings.

Sleep architecture matters more than total sleep hours. A patient sleeping seven hours but cycling through fragmented, non-restorative sleep will have a cortisol curve that looks like someone sleeping four. Wearable sleep tracking and, where indicated, formal polysomnography can clarify whether sleep is genuinely restorative or merely sufficient in duration.

Thyroid function deserves mention here as well. Subclinical hypothyroidism and autoimmune thyroid disease both create metabolic stress that drives HPA activation. Treating the thyroid abnormality often normalizes the cortisol picture without any direct intervention on the stress axis.

When Stress-Support Supplements Are Appropriate

None of this is an argument that adaptogens and cortisol-modulating supplements have no legitimate clinical role. For patients who have addressed the foundational variables — who are sleeping adequately, managing blood sugar effectively, have no significant inflammatory burden, and are experiencing genuine psychological or situational stress loads — there is reasonable evidence supporting the short-to-medium-term use of specific compounds.

Ashwagandha, in particular, has demonstrated statistically significant reductions in perceived stress and morning cortisol in several randomized controlled trials involving otherwise healthy adults. Rhodiola rosea has shown benefit in fatigue and burnout-related presentations. Phosphatidylserine has the longest evidence base for blunting exercise-induced cortisol spikes in athletic populations.

The operative phrase is "otherwise healthy adults." These trials were not conducted in populations with unresolved metabolic dysfunction, and extrapolating their findings to patients with complex, multilayered physiology is a significant inferential leap.

The Informed Patient's Responsibility

The wellness supplement market in the United States is, by design, consumer-driven. There is no gatekeeping mechanism that ensures a patient has ruled out the appropriate differential before purchasing a cortisol-support stack. That responsibility falls on the informed patient — and on the clinicians they work with.

Asking the right questions before reaching for a supplement is not a rejection of integrative approaches. It is the application of exactly the kind of evidence-based reasoning that separates genuine health optimization from expensive symptom suppression. When the stress response is persistently dysregulated, the most productive question is not which supplement will quiet it. It is what the body is responding to — and whether that signal deserves to be heard.
